Provider Revalidation Deadline
The Department has identified providers who have not completed required revalidation in accordance with federal regulations. A list of these providers is available at: Provider Information > Reports for Public Access > Provider Revalidation Past Due.

Effective July 1, 2026, providers who have not revalidated will be suspended and unable to participate in Georgia Medicaid/PeachCare for Kids® programs, including Fee-for-Service, Georgia Families, and Georgia Families 360. Claims with dates of service on or after July 1, 2026, will not be paid.

Suspension will be lifted once revalidation is completed; however, the effective date will be the application submission date. Retroactive enrollment is not permitted. Providers who do not revalidate within 30 days of receiving a suspension notice will be terminated and provided with appeal rights.

For members in Medicaid, PeachCare for Kids® or Georgia Pathways to Coverage™.

Georgia Families gives you choices for your health care! Choose a health plan and a Primary Care Provider for each member in your family who is in Medicaid, PeachCare for Kids® or Georgia Pathways to Coverage™.

As soon as you become a member of Medicaid, PeachCare for Kids® or Georgia Pathways to Coverage™ you will be enrolled into a health plan. If you do not want the health plan we chose for you, you can change to another health plan. You will have 90 days from the start date of your health plan to change. You can change your health plan in one of these three ways:

  1. Online: Go to Log In or Register link and follow the steps.

  2. By phone at 1-888-GA-Enroll (1-888-423-6765) Call Monday to
    Friday, 7 a.m. to 7 p.m.

  3. On paper, using a form you get in the mail from Georgia Families

1095-B Forms: Electronic Only Unless Requested

Due to a recent IRS decision, effective tax year 2020, form 1095-B will not be mailed unless requested. Form 1095-B is not required to file your taxes and the form should be retained with your tax records.

The electronic version of the 1095-B form is available by using the Log In or Register link. Log in with a valid member ID and password under the User Information panel. Members who have not yet registered for access may do so by following the Register for Secure Access link under in the Quick Links section.
